PRESENTATION:
Utilizing Social Determinants of Health to Improve Health Outcomes
Laura will discuss the five key social determinants of health and how deployment of social determinants of health interventions is shown to lower costs and improve health outcomes. Laura will then discuss how a Health Information Exchange may leverage social determinants of health data.
About Laura Young: Laura is an information technology expert with over 25 years working within the IT industry with a focus on government, non-profit, education and healthcare. She is currently the Executive Director for healtheConnect Alaska. Laura has been a leader in the healthcare IT sector for over 10 years with a specific focus on Clinical IT and Health Information Exchange (HIE) systems. Nationally, she is involved in a number of HIE and behavioral health interoperability efforts as the Managing Director of Converge Health Information Solutions. She serves nationally as the Vice Chair of the HIMSS Interoperability & HIE Community, as well as the eHealth Initiatives Connecting Communities Workgroup.
